Leon Forman, Ph.D,79, of Falling Waters passed away Saturday, October 20, 2018 at his home.
Born July 13, 1939 in Boston, MA, he was the son of the late Elizabeth Beatrice Freeman Lazarus and step father, Irwin Lazarus.
Leon was a scientist with Brookhaven National Laboratory. In 1987 he received the National Intelligence Distinguished Service Medal from the Department of Energy and in 1978 he was nominated for the E.O. Lawrence Award. He served in the US Army from 1966 to 1968 in the US Army Signal Corps. He was a member of Congregation B’Nai Abraham in Hagerstown, Maryland.
He is survived by his wife, Ruth Pauline Goldman Forman; two sons, Matthew D. Forman and wife, Mary Ann of Norman, OK and Jeremy S. Forman of Frederick, MD; two brothers, Albert Lazarus of Abu Dhabi, Stephen Forman and wife Elisa of Brockton, MA; one sister, Felice Lazarus of Maynard, MA; two brothers- in-law, Martin Brody of Kings Park, NY, John Chadwick of Washington, DC.; three nephews, Michael Brody and three children, Chase, Charlotte, and Asher of Harleysville, PA, Julian Brody and wife Polly and children Robbie and Jack of Southhampton, NY, Jonathan Lazarus and wife Katie and children Emma and Dylan of Norwood, MA; five nieces, Miranda Chadwick and spouse Kristy Noel of Washington, DC, Celia Chadwick of Arlington, VA, and Rebecca Chadwick and husband Scott Flaherty of Brooklyn, NY, Michelle King and husband Tony, son D.J. and wife Amber and grandson, Felix of Janesville, WI, and Joanne Whitson of the San Francisco Bay Area.
In addition to his parents he was preceded in death by one sister, Beverly Goldman, one niece, Ellen Rosen; and one sister in-law, Trudy Brody.
A memorial service will be held at a later date.
